An 18-item self-report scale developed by the World Health Organization to screen for attention-deficit/hyperactivity (ADHD) symptoms in adults.
Executive Functioning in Daily Life (Short Self-Report)
A 20-item self-report screen of everyday executive-functioning difficulties, based on the domains of the Barkley Deficits in Executive Functioning Scale (time management, organization, self-restraint, motivation, and emotion regulation).

A standard 21-item self-report scale that assesses the severity of anxiety symptoms.
Beck Depression Inventory
A standard 21-item self-report scale assessing the presence and severity of depressive symptoms.
Childhood Trauma Questionnaire (CTQ)
A 25-item self-report screen for difficult childhood experiences across five areas: emotional abuse, physical abuse, sexual abuse, emotional neglect, and physical neglect.
Difficulties in Emotion Regulation Scale (DERS-16)
A 16-item self-report scale that assesses difficulties in regulating emotions across several domains.
Dissociative Experiences Scale (DES-II)
A 28-item screening scale that assesses the frequency of dissociative experiences that may occur in everyday life.
Eating Attitudes Test (EAT-26)
A widely used 26-item self-report screening tool for attitudes and behaviors that may be associated with eating disorders.
Eating Disorder Examination Questionnaire (EDE-Q)
A self-report measure of eating-disorder attitudes and concerns over the past 28 days, covering dietary restraint and concern about eating, shape, and weight.
Epworth Sleepiness Scale (ESS)
An 8-item self-report scale that measures your general level of daytime sleepiness, or your 'sleep propensity' in everyday situations.
Insomnia Severity Index (ISI)
A brief 7-item self-report scale that assesses the nature, severity, and impact of insomnia over the last two weeks.
Mood Disorder Questionnaire (MDQ)
A 13-item self-report screening questionnaire for symptoms that may be associated with bipolar spectrum disorders.
Personality Inventory for DSM-5 (Brief Self-Report)
A 25-item self-report screen covering five broad personality-trait domains described in DSM-5: negative affect, detachment, antagonism, disinhibition, and psychoticism.
Pittsburgh Sleep Quality Index (Adapted Self-Report)
A self-report screen for sleep quality and sleep-related difficulties over the past month, adapted from the Pittsburgh Sleep Quality Index.
SCL-90-R Symptom Checklist
A 90-item self-report scale that screens a broad range of psychological symptoms (anxiety, depression, somatization, hostility, phobia, etc.).
Stages of Change Readiness and Treatment Eagerness Scale (SOCRATES)
A 19-item self-report scale assessing readiness to change alcohol or other drug use, across recognition of a problem, ambivalence, and steps already taken.
Yale-Brown Obsessive Compulsive Scale (Self-Report)
A 10-item self-report measure of the severity of obsessive-compulsive symptoms over the past week, covering both obsessions and compulsions.
Young Mania Rating Scale (Self-Report)
An 11-item measure of the severity of manic symptoms over roughly the past 48 hours. Originally clinician-rated; here adapted as a self-report screen.
